Oral Health America & Nat'l Anti-Spit Tobacco Campaign Pledge Sheet, 1999 8-1/2"W x 11"H
Used by schools as an 'official contract' for kids (mostly involved in youth baseball) pledging not to use spit/chew tobacco products.
Collector's Note: I've seen some slick dealers cut this sheet apart and sell them as Oral Health "mini-cards" or "promos". As you can see - they're not.

Additional Note: Some of these sheets can be found with a specific team logo (imprinted on top of the baseball next to "We Agree!") This suggests that at least some of these sheets might have been issued at team stadiums and team sponsored events, although the majority were likely distributed thru elementary school teams and/or Peewee League Baseball.
